permit & Driving Test

**TEEN PERMIT TESTING REQUIREMENTS**

1.DE964 Form (certificate from Alliance)

2. VOE from the attendance office at your High School OR Diploma/GED

3. Original U.S. Birth Certificate
    Passport with USA Visa
    Green Card
   (Only 1 of the above is needed)

4. Original Social Security Card- no photocopies

5. DL14 Application filled out in black ink and signed by Parent/Legal Guardian

6. Must present 2 forms of residency corresponding to the parent taking the student to DPS (i.e. Water, Gas, Electric, Cable bill)

7. $16.00 Fee Payable at DPS

Teen Driving Test requirements
